2023 Touch-A-Truck Waiver

Touch-A-Truck WAIVER and RELEASE I (print name) represent that I am the parent, legal guardian, and/or representative of ____________________________________________________________ (hereinafter “Child(ren)”). If I am not the parent or legal guardian of any Child(ren), I expressly represent that I have the permission of the Child(ren)’s parent(s) and/or guardian(s) to act as their agent in signing this waiver and release.

In consideration of participating in Touch-A-Truck (hereinafter “TAT”), sponsored by the Junior League of Chattanooga, Inc. on November 11, 2023 at the Warner Park in Chattanooga, TN, I agree to the following on behalf of myself, the Child(ren), my executors, administrators, heirs, assigns, and successors:

1. Waiver and Release of Liability I expressly understand that participation in TAT is at my and the Child(ren)’s own risk. I hereby forever release, discharge and hold harmless the Association of Junior Leagues International and the Junior League of Chattanooga, Inc. and all of its agents, volunteers, representatives, employees, and trustees (collectively, hereinafter “JLC”) from any and all liability, claims, demands, actions, and causes of action arising out of or related in any way to any damage, injury, expense or loss of any kind, including but not limited to any personal injury, death, and/or property damage, resulting from or related to (a) TAT; (b) any action or omission of the JLC, whether intentional or negligent during TAT; (c) any action or omission by me and/or the Child(ren), whether intentional or negligent during TAT; and (d) the publication of the likeness or image of me or the Child(ren) that is captured and/or recorded during TAT.

2. No Duty to Supervise I acknowledge that a parent or guardian MUST accompany and supervise the Child(ren) participating in TAT. I agree and understand that the JLC has no duty to monitor, control, or supervise me, the Child(ren), or anyone else during TAT. I agree to fully monitor, control, and supervise the Child(ren) during their participation in TAT, and that I am solely responsible for any injuries sustained or caused by the Child(ren) during TAT.

3. Minor Photo Release I give the JLC, its sponsors, vendors, agents, and subcontractors, and (b) The Association of Junior Leagues International, Inc. permission to publish in print, electronic and/or video format the likeness or image of the Child(ren) that is captured and/or recorded during TAT. By signing below, I agree that I have carefully read this waiver and release, fully understand its terms, realize that I am giving up substantial legal rights that I and the Child(ren) have, and sign it freely and voluntarily.

4. All ticket sales for TAT are final. TAT tickets are nonrefundable and cannot be returned or exchanged. All proceeds benefit the Junior League of Chattanooga.

2023 JLC Touch-A-Truck Photo/Event StatEment

Events sponsored by the Junior League of Chattanooga (JLC) may be photographed and/or video-recorded. Attendance at a JLC event constitutes the consent of all attendees, and the consent of the parents or legal guardians of any minor children in attendance, to the future broadcast, publication or other use of photographs or videos at the sole discretion of the JLC.

Further, you release the JLC, its officers, directors, employees, and agents from any and all claims, actions, and demands arising out of or in conjunction with the taking and use of said photographs and/or video.
